When a death occurs

When a loved one passes, it can be hard to know what to do next.

We understand that losing a loved one is an incredibly challenging and emotional experience. To help guide you through this process, we've outlined the essential steps you need to take.

Notify the proper authorities

If the death is unexpected, call 9-1-1 immediately. The medical team will guide you on what to do next.

If your loved one was under hospice care, contact the facility right away.

Contact us as soon as possible

Honor their end-of-life wishes (if known)

Check for documents regarding funeral arrangements, burial or cremation preferences, and organ donation wishes. See if the deceased named someone to handle these arrangements.

Important information

After the loss of a loved one, it is important to gather the following materials and documents.

Personal information

  • Full name and address at the time of passing
  • Next of kin name and address
  • Parents’ names including mother’s maiden
  • Level of education and work occupation
  • Marital status

Important documents

  • Social security number
  • Birth certificate
  • Will or letter of instructions
  • Military service records and discharge papers
  • Completed pre-planning guide with final arrangements (Not required)

Funeral arrangements

  • Clothing, including undergarments (no jewelry)
  • Recent photo of the deceased for identification
  • Music selections for the service
  • Prayers or passages for memorial
